DBS Volunteer Application Form
Thank you for volunteering to help at Laleham Church of England Primary School.
All volunteers at Laleham are required to complete a DBS application for the safeguarding of our children. The stages for the application process are as follows:
- Please complete the details below for your application to begin.
- Once the process has started you will receive an email from firstname.lastname@example.org where you will be asked to complete some further information.
- Please bring into the school office three forms of identification, one of these should be dated within 3 months, you will also need to make a payment of £10.50 for the DBS application fee that is charged to the school. You can do this via your child Arbor account, under School Shop.
- Once the process has completed which can vary in time length, you will receive a DBS certificate through the post. From the date of the certificate you have 30 days to register your application for the update system. We strongly recommend that you do register for the update service as this will avoid having to process a new application and your details can be used again for different services by providing your certificate number. The school are notified of your application completing.
